Revision Rhinoplasty in Asians
Wei J, Dai C, Li S.
What this paper says
A review noting that demand for revision rhinoplasty in Asian populations is increasing, driven by implant related complications, and that the patient's own tissue is central to rebuilding those noses.
Overview
The authors state revision rhinoplasty demand is rising in Asian populations. Complications occur frequently in practice, including implants shifting position, working their way out through the skin, becoming infected, causing the nose to contract, and skin death, after either a first operation or several. They state early detection and prompt appropriate management are essential to limiting the consequences.
Sections of note
- Review article; the abstract reports no patient numbers, no rates and no follow up.
- Listed complications are implant deviation, extrusion, infection, nasal contraction deformity and skin death.
- These are stated to occur after primary or multiple surgeries.
- Early detection, prompt management and appropriate treatment are described as essential.
- The authors state the patient's own tissues play an important role as support grafts for reconstructing nasal structure in revision cases.
What it means for a patient
- The listed complications are those associated with implants, which are widely used in this population.
- Skin death and nasal contraction are the most damaging on that list and are difficult to correct.
- Revision surgery in these cases relies on the patient's own cartilage rather than another implant.
- No rates, outcomes or techniques are reported in the abstract.
Why this paper matters
Implant based augmentation has been performed in very large numbers across Asia, so revision surgery for implant complications is a substantial part of current practice. This review sets out that pattern and the shift to autologous reconstruction. It supplies no data on how often these complications occur or how well repair works.
Terms
- Revision rhinoplasty: surgery on a nose that has already been operated on.
- Implant extrusion: an implant working its way out through the skin or lining.
- Nasal contraction deformity: scar tightening that shortens and distorts the nose.
- Skin necrosis: death of skin from loss of blood supply.
- Autologous tissue: tissue taken from the patient's own body.

Revision and secondary rhinoplasty
Between 5% and 15% of rhinoplasties are done again, and the published series say the second operation is a different problem from the first: the cartilage is gone, the skin is scarred and less able to survive, and the airway is usually part of the complaint.
Ethnic, Asian, Middle Eastern, African and Latino rhinoplasty
The nose in front of the surgeon decides the operation, not the patient's ancestry: skin thickness, cartilage strength and existing dorsal height set the limits of what any technique can deliver, and planning every nose toward one Caucasian ideal is the error the comparative data now names.
