Non-Surgical Rhinoplasty After Rhinoplasty: A Systematic Review of the Technique, Results, and Complications
Santamaría-Gadea A, Sevil-Serrano C, Buendía Pérez J, Mariño-Sánchez F.
What this paper says
Across 20 studies and 2,048 patients who had filler after surgical rhinoplasty, hyaluronic acid was used in 67%, middle-third irregularities were the commonest target, satisfaction was high, major complications were low, and skin necrosis risk was raised.
Overview
Surgical rhinoplasty has a high revision rate, and filler can sometimes correct residual defects instead of a second operation. The Spanish authors ran a PRISMA-compliant systematic review of adults with prior surgical rhinoplasty who then had filler, recording indications, complication rate, filler type and satisfaction.
Sections of note
- 20 articles, 2,048 patients.
- Hyaluronic acid in 67% of patients.
- Indications varied; middle-third (mid-vault) deformities were the most notable.
- High satisfaction across studies; low rate of major complications.
- An increased risk of skin necrosis was observed compared with filler in unoperated noses.
What it means for a patient
- Small defects after surgery can often be camouflaged with filler, avoiding a revision operation.
- An operated nose has altered blood supply and scar, which raises the risk of the filler damaging the skin; the injector should be experienced with post-surgical noses.
- Results are temporary.
Why this paper matters
It pools the evidence on a common but higher-risk use of nasal filler. Limits: heterogeneous observational studies, no pooled incidence figures given in the abstract.
Terms
- Non-surgical rhinoplasty (NSR): reshaping with injectable filler.
- Middle third: the cartilaginous mid-vault of the nose.
- Skin necrosis: skin death from blocked blood supply.
- PRISMA: reporting standards for systematic reviews.

From the abstract
“Surgical rhinoplasty is a complex procedure with a high revision rate. Nonsurgical rhinoplasty (NSR) could avoid secondary rhinoplasty allowing the correction of postsurgical defects. A systematic review has been performed among adult patients who had previously undergone surgical rhinoplasty and now presenting for…”

Non-surgical and filler rhinoplasty
Injecting hyaluronic acid into the nose takes fifteen minutes, satisfies almost everyone who has it, lasts eight to fourteen months, and carries one risk that no amount of satisfaction offsets: filler entering an artery, which in 22 published cases took the patient's sight and gave it back to only 27% of them.
Complications and management
Serious complications after rhinoplasty are rare: infection runs under 1% in the two largest single-surgeon series, pooled rib harvests record no pneumothorax, and the bleeding series record no thromboembolism.
